Polices

Please read fully and carefully before booking.

We have policies in place to promote a comfortable environment and to show respect to our patients and staff. We ask that you treat all our staff with respect and decency upon the enforcement of our policies.

If you are not comfortable abiding by our policies, please do not move forward with booking.

 

No extra guests are to accompany you during the appointment unless they are getting serviced as well. – NO EXPECTATIONS!!!!!!!!!

  • This includes no children, as I do not service minors. Please do not bring any guests to your appointment, especially children as this poses a safety risk.

 

CANCELLATION POLICY

We at Unique Esthetics, are very dedicated to providing you with the best care possible. For that reason, your appointments is reserved specifically for you with plenty of time to discuss concerns, ask questions or get an amazing treatment with one of our highly trained providers!

Same day cancellations or no call, no shows will result in a non-refundable deposit at the time of your next booking.

 

TWO cancellations will result in 50 % of service required at booking.

THREE cancellations will result in an inability to book with Unique Esthetics in the future.

TWO ​no call no shows will result in an inability to book with Unique Esthetics in the future.

All Appointments with our RN, NP, DNP will require a 48-hour cancellation notice. Deposit will not be refunded and a 50$ cancellation fee will apply prior to rebooking.

 

Model Days you must cancel at least 72 -hours prior to date, if you do not you will not receive deposit.

Emergent cancellations will be discussed.

Please be considerate and respectful of our Policies.

LATE POLICY

We will give each client a 10 min grace period after that you will be changed an additional 25$.

 

 If you are more than 15 minutes late your appointment you may be cancelled, and you will need to reschedule.

ADDITIONAL INFORMATION

Once you book we will reach out to you to confirm your appointment, we will request a picture of your lips to be sent prior if getting lip filler.

If getting Tox we will ask you for pictures of you making a mean face, smiling and raising brows. We will set up your appointment with NP.

VIRTUAL ASSESSMENT

Prior to any Aesthetic Services

 A good faith exam is a medical evaluation that a licensed healthcare provider performs to determine if a procedure or treatment is appropriate for a patient. It's also known as a pre-authorization exam, clearance exam, initial exam, or physical exam. Our NP will go over medical history, allergies, any previous services. She will go over how procedure is done step by step.  She will go over any side effects and aftercare instructions. We want you to ensure that you are educated thoroughly prior to your service.

 

You will receive a link for your Virtual Assessment to be completed with one of our Highly Trained NP. {Ashley, Jenna, or Ebony} prior to your appointment. We typically aim to get it completed 24hours prior to your appointment. Virtual Assessment/ GFE is needed prior to service. Please log-in at least 10 minutes early and make sure you have access to good internet. If you are having problems with the link, please reach out to our Spa Assistant Christine 330-906-9763. 

If you miss you GFE with our NP there will be a 30$ rebooking. We personally reach out to you to ensure that we have the perfect time selected by YOU.

CONSENT FORMS

All services performed by Registered Nurse, REQUIRE you to fill out consent forms, they will be in Aesthetic Records, all consent forms MUST be completed prior to appointments. If you have question, please contact Christine our Spa. 

Please have your forms filled out prior to your scheduled appointment time. If you are unable to fill out forms prior to your scheduled appointment please arrive 10 minutes early. If forms are not filled prior and you do not arrive to the office 10 minutes prior to your appointment. Your appointment may need to be rescheduled and a fee of $50 will apply.

How do I get Started

Medical Weight Loss Injections are weekly injections. To help promote weight loss. 

1. Make appointment and pay deposit (your deposit covers your labs and your assessments by RN and NP). 
2. Once appointment is made, we will use your address you place on profile to send you to the closest lab collector. { please complete profile in FULL: Full name, address, DOB, email and phone number}
* Once your labs are completed your deposit is none REFUNDABLE
3.Balance is paid at the time of in person appointment. 
4. Client is responsible for booking appointments via website or app
What are my option on administration?
A.  Weekly Option: More Costly
-client has no commitment and pays weekly 
B.  Monthly Option:
-client is on the 12-week commitment will come in weekly for injections 
C. Hybrid Option: 
Hybrid Option:  self-administer at home.

-client is on the 12-week commitment
Labs Required: CBC, CMP, Lipid Panel, TSH,
•    If BMI is over 30 additional labs are required.
o    CRP, Cortisol Level.
•    Labs must be done prior to starting injections
Virtual Assessment: Must be completed by NP
In person Physical Assessment: 

We also have packages with multiple services to help w/weight loss.

How to qualify?
Must be in the state of Ohio, must have a BMI greater than 30, or greater than 27 with hypertension, high cholesterol, or diabetes type 2 or other co-morbidities.

Who DOES NOT qualify?
No history or family history of thyroid cancer, or thyroid tumors, no type 1 diabetes.

DEPOSITS

Everyone is required to pay a deposit when .No deposit! No appointmen! 

PAYMENTS

We take cash, FSA/HSA, all credit cards.

We also have payment plans through Cherry and Care Credit.  Unfortunately if you are using one of these forms of payment we cannot honor special pricing, you would be responsible for the FULL PRICE.

By choosing to book, you are agreeing to all these policies.

REFUNDS: SERVICES

We do not offer refunds on services rendered. Once services are purchased, they will not be refunded. However, to ensure our clients always receive the greatest experience, unused service values (cash equivalent for the remaining amount of a treatment package) can be applied to any other service at Unique Esthetics.

All injectable treatment sales are final; refunds or credits cannot be offered once treatment is completed.

During botulinum toxin appointments, our Nurse Practitioners assess the facial muscles and decide what number of units are sufficient for adequate treatment. This can vary from patient to patient and from treatment to treatment. To provide the best level of patient care, we recommend you follow up with an appointment or call 10-14 days after your botulinum toxin appointment if further treatment is needed. If additional units are needed, we will require additional payment.

REFUNDS: PRODUCTS

Unopened products may be returned within 10 days of purchase with the receipt. In the event of an unexpected adverse reaction from a product, store credit is available only upon consultation with your provider. Item must be returned in-store with proof of purchase within 30 days of purchase.

SHIPMENT OF PRODUCTS

Convenience is key, and we’re happy to offer shipping for our medical-grade skincare! Shipping starts at $12 and may increase based on order size or weight. If you require a signature or insurance, this will require an additional fee. We release responsibility for shipments that are lost, stolen, or otherwise damaged after the order has shipped from our office. We do not offer refunds or free products if an order has been lost, stolen, or damaged from shipment. If your order has been lost, stolen, or damaged, we encourage you to reach out to the United States Postal Service to file a claim.​​

REFERRALS 

If you refer ONE client to Unique Esthetics, you will get 10% off your next service.

If you refer TWO clients to Unique Esthetics, you will receive 20% off your next service.

If you refer THREE clients or more and you will receive a free service.​

WHAT IS A SEAT FEE

All services done by RN, DNP, NP will require a seat fee of 100$ 

Required for raffle winnings ​

ALL SERVICES MUST BE FULL PRICED, NO SPECIALS, PROMOTIONAL OR EVENT PRICED SERVICES TO GET DISCOUNTS AND FREE SERVICE. 

HOME SERVICES

We do offer home services with a flat fee of 50$ plus mileage.{ fee subject to change}

We require you to put all pets up during appointment. 

Please ensure that our staff is coming into a safe and violence free home.

UNDER 21 YEARS OF AGE

Parents/ Guardian you must sign a consent to treat form for all aesthetic services. 

Parents/ Guardian you are required to take a copy of your ID for your chart to confirm identity.

Lip fillers are FDA-approved for patients over 21 years old, but patients over the age of 18 may receive this treatment.

Patients under 21 may be able to receive filler with parental consent.

We can do a virtual consent with parent and or legal guardian.
